Welcome to Jackie's Embroidery Connection Order Form. To use this form please browse our Design pages. Write down the item number and cost of each design you are interested in ordering. Then return here and insert these values into the order form. If you are using this form to pay for digitizing fees then please fill in the amount charged into the "digitizing fees" block right above the "sub-total" block. Total the cost of your order and type your total cost into the "Total Due" field. Also be sure to fill in the "Your E-mail Address" field. Then please PRINT OUT your completed Order Form. Mail this Order Form along with your check or money order payable to: Jackie Clagett to the address below. Designs will be e-mailed to you upon receipt of your order and payment.
